Will my insurance cover the cost of an ALCAT test?
Yes, certain insurance policies will pay for all or part of Alcat testing.
*Patients must have out of network benefits. NO HMO’s are accepted.
Both Medicare and Medicaid do not cover this test.

This matter can only be addressed between your Provider and yourself, As every Insurance Company is different in the coverage that they provide. Healing Arts For Wellness and QEM strongly advises that you inquire directly with your Insurance company to verify what their specific coverage is in regards to Food Sensitivity Testing (CPT code 83516- Antigen Allergy Test). Regardless of your coverage Healing Arts For Wellness will need to be paid for in full prior to testing. CareCredit financing availible.

- Does the plan provide “out of network laboratory benefits?
- Is there an out of network deductible? If yes, how much?
- Is CPT code 83516- Antigen Allergy Test covered?
What do you need to file an insurance claim?
- A full payment for the Weight Loss Program, You will receive a receipt to submit to your Insurance Company for reimbursement.
- A prescription from a physician which includes an ICD-9 diagnosis code, along with their Tax ID# or UPIN # included.
Are there any insurance companies who do NOT cover the ALCAT test?
- United Healthcare limits coverage to 4 units per day – our test bills a minimum of 10 units in one day. They may pay a portion of the total.
- Blue Cross of Michigan- for the same type of unit limitation per day
- Aetna -due to non-payment of claims.
Both Medicare and Medicaid do not cover this test.
